Bayfield Transition Services
Services for Students Ages 18–21
The Bayfield Transition Services provides individualized services and supports for students with disabilities ages 18–21 as they prepare for adult life beyond high school. Transition services are designed to support each student’s movement toward greater independence in employment, education and training, community participation, and daily living.
The program is guided by the requirements of the Individuals with Disabilities Education Act (IDEA) and is based on each student’s strengths, interests, preferences, and individual transition goals identified through the IEP process.
Program Vision
The Bayfield School District believes all students should have opportunities to develop the skills necessary for meaningful participation in their communities and successful adult lives. The Transition Program supports students in building independence, self-confidence, self-advocacy, and real-world skills through individualized and community-based learning experiences.

Employment and Career Readiness
Students may participate in:
- Career exploration
- Job readiness training
- Resume and interview preparation
- Workplace communication and social skills
- Job shadowing and internships
- Supported work experiences
- Vocational skill development
Independent Living Skills
Instruction may include:
- Budgeting and money management
- Cooking and nutrition
- Personal hygiene and self-care
- Household and organizational skills
- Time management
- Transportation and mobility skills
- Community access and participation
Postsecondary Education and Training
Students may receive support related to:
- College and technical school exploration
- Disability support services
- Applications and enrollment processes
- Workforce training opportunities
Self-Determination and Self-Advocacy
Students are encouraged to:
- Participate in IEP meetings
- Set personal goals
- Make informed decisions
- Develop problem-solving skills
- Understand accommodations and supports
- Build confidence and independence
Community-Based Learning
A major component of the Transition Program is community-based instruction. Students may participate in real-world learning opportunities in school and community settings that support individualized postsecondary goals.
Community-based experiences may include:
- Employment training sites
- Recreational and leisure activities
- Grocery shopping and banking
- Public and community transportation
- Volunteer opportunities
- Accessing community resources and services
These experiences are designed to help students apply skills in authentic environments while building independence and confidence.
Student Safety and Support
The Bayfield School District prioritizes student safety while promoting increasing independence and responsible decision-making.
Students participating in community-based instruction and transition activities receive individualized levels of support and supervision based on their needs, abilities, and goals. Staff provide instruction, modeling, coaching, and support to help students safely navigate school, work, and community environments.
Safety instruction may include:
- Community and pedestrian safety
- Transportation safety
- Workplace expectations and safety
- Personal boundaries and healthy relationships
- Emergency response and communication skills
- Digital and social media awareness
Collaboration and Partnerships
The Transition Program works collaboratively with:
- Families and guardians
- Community employers
- Adult service agencies
- Vocational Rehabilitation
- Postsecondary institutions
- Community organizations and support providers
Partnerships help students connect to adult services, employment opportunities, and community resources prior to exiting school services.
Eligibility
Students receiving special education services through an Individualized Education Program (IEP) may be eligible for transition services through age 21 if they continue to demonstrate transition-related needs and have not received a regular high school diploma.
Eligibility and services are determined through the IEP process and are based on individual student goals and needs.
