Enrollment Packet

Welcome to Bayfield High School.  Please click the LINK below to download the registration packet or stop by the school and we can print one out for you! If your student is new to the district, you may need to provide the following documents.

• A copy of your child's birth certificate
• A copy of your child's most current immunization record
  • (If your child is exempt, we have a form for you to sign.)
• Proof of Residency
